Snap a photo of your receipt and send it by text message or email to cs@drvsusa.com by 11:59:59pm PT 12/31/14. Image MUST contain retailer name, date, time, receipt total, & three (3) qualifying purchases. If your receipt is too long to capture all these details in the photo, please fold your receipt prior to taking a picture, and make sure that the following details are visible: retailer name, date, time, receipt total, & three (3) qualifying purchases.
Follow the link sent to you & enter your information to receive a $3.50 manufacturer’s reward certificate via US mail.
